Manawatu Hearing Aid Fund

Applications are open all year round.

 
Purpose
To subside the cost of hearing aids for residents in the Manawatu region.
 
Conditions
Applicants must reside in the Manawatu region at the time an application is made.
 
What does it cover?
Maximum of $500 per person which is paid directly to the applicants nominated Audiology provider and can be in addition to any other subsidies given to an individual who is hard to hearing that reside in the Manawatu region at the time the grant application is made.
 
Individual applicants are required to:

  • be a New Zealand citizen or have permanent residency.

  • be hard of hearing and require hearing aids.

  • submit ALL supporting documents, along with a fully completed application form by the closing dates specified below, or your application will NOT be considered.

 
Supporting documents required:

  1. A recent audiogram and supporting letter from your audiology provider.

  2. A scanned copy of a pre-printed deposit slip or bank statement which includes the full bank account number (bank, branch, account number and suffix) and the account holder's name. Must be signed and stamped by the bank within the last 90 days.

  3. Proof of NZ citizenship or residency, in the form of a passport or birth certificate.

  4. Accountability report for ANY previous funding received from the NFD Trust (if applicable).

  1. Fully complete the application form below and ensure ALL supporting documents are attached.

  2. Return application and supporting documents by the closing date.
